A postcard size piece of card titled 'Press your point' Customer Comment Card. It has a 7 questions, with 4 to 6 short answers, each printed inside semicircular perforated areas which the customer must press and tear to indicate their answer.

What an amazing little idea from the internet era, but just before smartphones ubiquity, easily created webforms, QR codes, and relentless requests for reviews and feedback from every company and delivery services you interact with.

I never filled one out. I just grabbed this for interest in the archive. I bet it would still work well today. In fact, the novelty and effort might increase engagement.

Although it does invite potential of hanging chads, and history tells us how consequential they can be!
